Observer's Nightly Report Form

All MDM observers are required to fill in the form below with a brief summary of the night's observing activities.

Please read the submission instructions before filing a nightly report for the first time. You can also get detailed information on the various form entries by clicking on the individual highlighted items. If you use the help facility, remember to use the "Back" button on your browser to return to the form in progress (otherwise your entries up to that point could be lost).

If you encountered any hardware or software problems during the night, please remember to also submit a separate Trouble Report.
